Booking An Appointment On Behalf Of A Second Party?
If you are booking an appointment on behalf of a second
party you will be required to supply your full name, mailing
address and telephone number. We will also require that your Credit
Card and valid drivers license numbers be kept on file.
If you are booking an appointment on behalf of a second party we will
return your call to verify that you are calling from the telephone
number you have provided us with.
If you are booking an appointment on behalf of a second party then you
agree you will assume full responsibility to cover the bill in the
event that the party requiring service does not arrive at the
address you requested service for.
